What are the common side effects?

Nausea, vomiting, diarrhea, headaches, loss of appetite, constipation, GERD, changes in menstruation cycle have been seen. Every person is different and they may have some of the side effects or not at all. Some may have with every dose or every dose increase. 

Constipation: take Miralax once a day if you are prone before starting. If constipation persists, it can be taken twice a day. If no relief has been achieved a glycerin suppository, Magnesium citrate or oxide, Epsom salts, or enema can be used. Once stools have returned or softened, medication should be reduced to Miralax once a day and Colace once a day. Magnesium 350mg/day also helps. A liquid diet may be needed until stools return to a normal pattern.

Diarrhea: Eating bulk foods such as rice, bananas, bread/toast, potatoes, plain crackers, aged soft cheeses, cottage cheese, yogurt, green olives, dark chocolate, sourdough bread, pickles and applesauce can help bind stool or are a good source of probiotics. Probiotic medication or glutamine supplements can also help. Pepto-Bismol, Kaopectate or Imodium can also be used.

Headaches: The use of acetaminophen or ibuprofen can be used. Alternative methods include massage, cold compress or a heating pad.

Nausea: Eating raw ginger or placing in water, peppermint aromatherapy, eating bland foods like crackers or toast, drinking tea, medications like Zofran, Dramamine or Reglan can help ease symptoms. I have 4mg and 8mg Zofran for purchase if needed.

GERD: Heartburn and indigestion have been known to occur. Taking a daily medication like omeprazole or esomeprazole magnesium can help. Also the foods you eat will cause this as it sits in your stomach for long periods of time.

Loss of appetite: This is how the medication works, by telling your brain that you are not hungry. The best way to combat eating enough food is to eat protein bars or drink protein shakes. No Cow, RXBar, Good!, Legendary, Kind, and Aloha have 12g or higher in protein. Slate, Core Power, Boost, Soylent, Iconic, Muscle Milk, and ISO100 are some good options.
You can, just don’t expect great results. Since you will not be eating as much due to less appetite and slowed gastric motility, these are where your calories are going to come from. Vitamin D at 32-36 IU/lb is recommended to assist in weight loss and maintenance. Max dose is 4000IU/day. Best diet is the Mediterranean Diet for weight loss and maintenance.

You do not have to exercise but this is a great time to begin a new lifestyle choice. You will still lose weight, just not as quickly and you will regain back if you stop taking medication if a change in lifestyle is not made. Just adding walking at an incline is a great way to make changes. As you lose weight, it will become easier. Vitamin B12 at 4.2mcg/day also helps with increased energy to help you.

  • Losing weight to fast, over 5 lbs in a week, can be cause for concern as you may be losing it too fast.
  • Pancreatitis is also another concern. This is mostly described as a very sharp (severe) pain on the left side of your back that may spread to your chest and can feel worse after eating. You may also have fluid buildup in your stomach, fever, nausea, and vomiting with the pain. It is recommended to see your PCP if you experience any of these symptoms.
  • This medication has been shown to cause thyroid cancer in rats. Any family history of this should be reported during your head to toe assessment as it can increase your risk of developing this disorder.
With lifestyle changes, you may not have to be. Some people find that they only need 5 units of Semaglutide or 12.5 units of Trizepatide every other week. Every person is different.

I titrate you up and down on this medication. So if you are losing weight at 20 units, we do not need to increase your dose until you plateau at a weight for 2 weeks. Since hormones and what you consume affect your weight, it is always best to ensure that you have truly plateaued before moving you up in dosing.

If you have not lost any weight at your current dose for over 2 weeks, we can move you up to the next dose increase. We want to ensure that you are not increase too fast to cause any issues and ensure you lose weight safely.

Most people do not see any significant weight loss until 20 units. However, we want to ensure that you are not increasing your dose at an unsafe rate, that is why the dose is started off at 5 units for Semaglutide and 12.5 units for Trizepatide.

Yes, for a nominal fee we can store and inject your medication for you. However, many people feel this way initially and after the first injection take a syringe home to try for themselves or have a family member inject them. We also offer sublingual.
Semaglutide and Trizepatide need to be stored in the refrigerator. The bottom shelf in the back is the best place, the doors are the worst place.
Some airlines and cruises will only allow you to take medication if it is in the original drug package container to identify the medication. Since it is liquid and needs to be on your carry on, it is best to place your vial in the original carton in a separate bag for easy inspection for TSA. You should also have a portable medical cooling device for your travels (this can be purchased from Amazon or a pharmacy).
We can also have you adjust your injection schedule if needed to take the night before or morning of your departure and the day you return- this is the best option if you will be gone for a week to 10 days.
If you missed your scheduled dose and your next dose is more than 48 hours away, inject your medication as soon as possible. If your next dose is less than 48 hours away, skip the missed dose and inject according to your next scheduled dose. If you miss two weeks of doses, you may need to restart the medication at the lowest dose and titrate up again to the doses you were at. This is the safest way to begin your medication again.
Remember if you do take your medication with you, talk to cruise staff to ensure that you will have access to a refrigerator to store your medication.

Remember that protein is the most important food you can eat along with plenty of water- ½ your body weight in ounces.
Salt intake will cause water retention and slow down weight loss, add 8 oz of water for every tsp of salt.
Weigh yourself weekly, same day, time and attire. I will need to see this log at least every month.
Please discontinue medication 7-10 days prior to any surgical procedure and share this with your anesthesia.
